Faculty Record Sheet
An annually updated record of each faculty member's professional activities is required by the Policy of Appointment, Reappointment, Promotion, and Tenure (IV.A.1).
Faculty Record Sheet Online Form (2022-23 & 2023-24)
The completed Faculty Record Sheet (FRS) form along with an updated CV is due September 15, 2023. Please provide a full listing of your activities in the areas of teaching, scholarship and service for the 2022-23 academic year. If you have trouble logging in, please call the IT Service Center at 413-585-4487.
The FRS is the primary document used to determine merit pay increases for senior lecturers, associate professors, and professors. Copies of your FRSs are distributed to your chair.
CVs will be used for reference throughout the year by our office, other Smith offices, visiting committees, students, parents, alumnae and others interested in learning about Smith faculty members.

Why do you need my CV?
The Provost's office receives a range of requests for faculty CVs throughout the year and so it's important for the office to have an updated version of each active faculty member's CV on file. Examples of requests include: as part of a decennial review (departments/programs need to undergo review every 10 years), from the Office of Corporate & Foundation Relations to determine grant eligibility, as well as other offices on campus for additional background information on faculty.
What if I am unable to log in?
Please make sure you are entering your username only and not your full email address. If you are unable to log in, please call the IT Service Center (x4487) and let them know you are having difficulty logging into the FRS form with your Smith Network username and password.
How do I save a copy of my FRS?
You can save a copy of your FRS by creating a PDF version. Go to the online form, sign-in, and select the appropriate year. When your FRS opens, click "Print this page" and then select Adobe PDF as your printer.
